PATIENT INSTRUCTION FOR IMMIGRATION PHYSICAL
1. Please bring a completed Immigration Form I 693 filled out in black ink.
2. Patient’s must bring the following:
• Valid Identification Card (acceptable government ID’s- passport/photocopy of
passport, work permit, boarder paper with photo, license)
• Vaccine Records (including childhood vaccines, recent vaccines and covid vaccine
records)
3. For Non-English Speaker patients,
• Must bring own translator/interpreter who speaks fluent English and their language
4. Patient will have 2 visits in the office.
First Visit (Nurse Visit)
• This visit is to obtain all the patients records including vaccine records and blood
work-up
Second Visit (Doctor’s Appointment)
• This is done 2 weeks after the first visit.
• The doctor will discuss with the patient all the lab results and completion of the
paperwork.
5. In an event that a patient decides to get their vaccines outside of Stetho MD, they will
need to bring in the following details before their second visit with the physician for a
physical:
• Lot #
• NDC #
• Manufacturer #
• Expiration #
• Date given
6. Immigration Physical $440. (Cash Only and Non-Refundable)
• Labs are included
• Vaccines not included
